REGISTRATION


Under normal circumstances, the doctor’s certificate giving the cause of death should be taken to the Registrar Office with if possible the deceased’s medical card.


PERSONS QUALIFIED & LIABLE TO ACT AS AN INFORMANT

DEATHS IN HOUSES AND PUBLIC INSTITUTIONS

A relative of the deceased present at the death.

A relative of the deceased in attendance during the last illness.

The occupier if he/she knew of the happening of the death.

Any inmate if he/she knew of the happening of the death.

The person causing the disposal of the body.


DEATHS NOT IN HOUSES OR WHERE THE DECEASED HAS BEEN FOUND

Any relative of the deceased having knowledge of any of the particulars required.

Any person present at the death.

Any person who found the body.

Any person in charge of the body.

The person causing the disposal of the body.


FROM THE REGISTRAR

Obtain the GREEN CERTIFICATE which is required by Calverton Funeral Service.

Purchase copies of the entry of death as required for legal and insurance purposes.

Certificate of Registration/Notification of Death White Form BD8 for Social Security

purposes (if applicable).


DETAILS REQUIRED BY THE REGISTRAR

The procedure for registering a death is a simple question and answer interview between the Registrar and the informant. The following may be required.


For deceased men and women (all cases)

Death Certificate,  Medical Card,  Full name,  Home address,  Date of death,  Place of death, Date of birth,   Place of birth,  Last occupation, Date of birth of surviving partner, 

Burial or Cremation.

Is deceased in receipt of pension or allowance from public funds.


For deceased married women (all cases)

Maiden name,  Husbands full name,  Husbands last occupation (even if deceased).


For the informant

Full name,  Home address,  Number of copies required.

Phone lines are open 8am to 8pm Monday-Friday and 8am to 12pm on Saturday.
